Genetic generalized epilepsy: factors associated with drug resistance polytherapy

چکیده
Abstract Background Different studies investigating generalized genetic epilepsy (GGE) have shown that achieving 5 years of remission, early seizure and needing antiseizure medication (ASM) monotherapy may predict favorable long-term outcomes. Results This is a retrospective analytical cohort study. Records patients with GGE diagnoses at large center in Cairo served as the data source. 630 (297 male (47.1%) 333 female (52.9%)) were included, their median onset age was 13 years. The follow-up period this study least 4 418 (66.1%) remitters, 160 (25.4%) late 52 (8.3%) intractable. In addition, 367 (58.3%) needed single ASM to achieve maximum remission (monotherapy group), while 263 (41.7%) polytherapy. Stepwise regression analysis revealed absence seizures, clusters frequency before treatment, dose sodium valproate (VPA) until first independent predictors for Moreover, VPA dose, catamenial seizures females intractability. Conclusion majority outcome, some clinical features could need polytherapy failure on treatment.

Genetic factors associated with drug-resistance of epilepsy: Relevance of stratification by patient age and aetiology of epilepsy
Epilepsy drug-resistance may depend on the metabolism of antiepileptic drugs (AEDs), transport to the epileptic focus and/or target sensitivity. Furthermore, drug response depends on multiple characteristics of the patient, the epilepsy, and the antiepileptic drugs used. We have investigated the association between polymorphisms related to antiepileptic drug metabolism (CYP2C9, CYP2C19, and UGT...
متن کاملMonotherapy or polytherapy for epilepsy?
Monotherapy has been promoted as the ideal in epilepsy treatment because of reduced side effects, absence of drug interactions, better compliance, lower cost and, in many cases, improved seizure control compared to polytherapy. The question of monotherapy vs. polytherapy has assumed increasing importance with the availability of multiple new antiepileptic drugs (AEDs), initially tested as add-o...

PURPOSE To estimate the direct costs associated with the current management of focal epilepsy in adults treated with a combination of antiepileptic drugs (AEDs) in France and the supplementary costs of drug resistant epilepsy as defined by the International League Against Epilepsy (ILAE) in 2009. METHODS ESPERA was a multicentre, observational, cross-sectional study conducted in France in 201...
متن کاملAdverse effects & drug load of antiepileptic drugs in patients with epilepsy: Monotherapy versus polytherapy
BACKGROUND & OBJECTIVES Although the need for a combination of antiepileptic drugs (AEDs) in the treatment of epilepsy is well justified, but an associated increase in adverse effects (AEs) lends a restriction to polytherapy. The aim of this study was to evaluate AEs and drug load (prescribed daily dose/defined daily doses) of AEDs in patients with epilepsy (PWE). METHODS Consecutive PWEs att...
متن کاملClinical factors of drug resistance in juvenile myoclonic epilepsy.
Juvenile myoclonic epilepsy is a comparatively benign form of idiopathic generalised epilepsy. Little is known about the prevalence of difficult to treat or drug resistant patients. Among 155 consecutive patients with newly diagnosed juvenile myoclonic epilepsy evaluated between 1981 and 1998 and followed up for at least 1 year (61 men, 94 women; aged 15-70 years, mean 33 (SD 10.3); onset of ju...
